1. Birth Certificate

2. Baptismal Certificate (only if the child is Catholic)

3. School Records: Standardized test scores (students in grades 2-8 need most recent test scores), and report cards (2006-2007 and final report card for 2007-2008)

4. School Health Certificates: Students are NOT admitted unless up-to-date physical and dental certificates and immunization records are on file in the school office by August 1.
5. If your child will participate in CYO basketball or any other sport, the physician must check the appropriate box on the student's physical form. | |
